REGISTRATION FEES, PAYMENT & CANCELLATION POLICIES

The online registration has been closed. Please register ON-SITE directly at the congress venue between June 27 and June 30, 2009.

Please note: If you are submitting an abstract please do this before registering. After your abstract submission you will be sent an access code which will enable you to register for the congress.

Please fill in one congress registration form per delegate and accompanying person.
You will receive a personalized badge when collecting your registration documents. During the Congress, this badge must be clearly visible at all times and will grant you access to the Scientific Sessions and the Commercial Exhibition.

For group registrations (more than 10 persons), we kindly ask you to contact MONDIAL CONGRESS & EVENTS to arrange details.

REGISTRATION FEES

* Please note: Residents will have to provide a resident document beforehand when registering. If the delegate who has registered as a resident does not provide a valid confirmation of residency regular fees will be charged.
** Please note:  A copy of your ID will be required to prove the year of birth.
*** Please note: Bookings will be available from December 2008 on. Delegates already registered will be informed via E-Mail when Instructional Courses are available and can then book them, too.

Participants' registration fees includes:

• Admission to all Scientific Sessions (except Instructional Courses!)
• Admission to Poster- and Commercial Exhibition
• Conference materials (congress bag, final programme, abstract publication, badge)
• Opening Ceremony
• Certificate of attendance

Accompanying person's registration fee includes:

• Opening Ceremony
• Admission to general areas of the Congress Center (not to Scientific Sessions!)

Please note that registrations can only be accepted and confirmed upon receipt of the completed registration form (mailed, faxed or filled in via the website www.eaorl-hns2009.com) and receipt of full payment. For information on payment, please refer to "Terms of payment".

The date of the remittance order (as stamped on the slip) will be decisive for early-bird and regular registration.
As soon as the Organising Secretariat has received your payment, a letter of confirmation will be mailed. Confirmations  can only be mailed if an e-mail address is provided on the registration form. This option is highly recommended as it helps avoid time consuming correspondence. Otherwise, confirmations will be faxed or land-mailed.

TERMS OF PAYMENT
The total amount outstanding should be made out in advance in EURO (payments in other currencies will not be accepted) in favour of 1st Meeting of the EAORL-HNS and only by one of the following procedures:

Only Visa, Eurocard/Mastercard and Diners will be accepted.
The cardholder´s name, card number, expiration date and an original signature must be on the booking form.

Comment: "Free of charge for MONDIAL CONGRESS & EVENTS" For transfers from outside of Austria please use IBAN (international bank account number) and BIC (bank identification code) to minimize bank charges.
Bank fees will be charged to the participant.
Mondial Congress & Events shall act as mediator only and cannot be held responsible for any loss incurred or any damage inflicted on persons or objects irrespective of whatsoever cause. The liability for transport and other service companies shall not be affected by the above. Only written agreements shall be valid. The place of jurisdiction shall be Vienna.

CANCELLATION POLICY
Refunds - less EUR 40,- administration fee - will be granted to delegates unable to attend, provided written notice is received by MONDIAL CONGRESS & EVENTS by April 27, 2009. After this date, no refunds are available. Amounts due will be reimbursed after the Congress.
Please present your letter of confirmation at the registration desk upon arrival.


TRAVEL GRANTS
The  1st Meeting of the EAORL-HNS aims to provide all ENT colleagues from Europe and beyond the possibility to attend this event. Extensive travel grants will be supported by the EAORL-HNS and its subspecialty societies as well as by the industry. In order to be eligible, applicants have to fulfil the following criteria:

• Applicant must submit an abstract
• Age of applicant is 35 years or below
• Preference is given to applicants from Eastern European Countries
